Mu-opioid receptor (MOR)

A G-protein-coupled receptor in the central and peripheral nervous system. The main binding site for endogenous endorphins, classical opioids, and the kratom alkaloids mitragynine and 7-hydroxymitragynine.

Why it matters

MOR activation produces analgesia, sedation, euphoria, and — in the case of full agonists like fentanyl or morphine — respiratory depression. The receptor is found densely in pain-processing regions of the brainstem and spinal cord, in reward circuits like the ventral tegmental area, and in the gut (which is why opioids slow digestion). Different ligands activate MOR with different downstream signalling profiles, and that profile dictates the risk landscape of a given compound.

In practice

Mitragynine binds MOR as a partial agonist with a “biased” downstream profile — it preferentially activates G-protein signalling (pain relief) over beta-arrestin signalling (respiratory depression, tolerance, dependence). That bias is the leading hypothesis for why kratom has been associated with a different overdose risk profile than classical opioids in epidemiological data, though it does not mean kratom is risk-free.

Common confusion

“Partial agonist at MOR” is sometimes read as “not really an opioid.” The clinical reality is more nuanced: MOR ligands cluster on a spectrum of efficacy, and partial agonists can still produce dependence, withdrawal, and dangerous interactions with other CNS depressants. Kratom is opioid-adjacent, not opioid-free.
